Vaccinations

If you require any vaccinations for foreign travel you will need to complete a travel questionnaire, which includes information about which countries and areas within countries you are visiting to determine what vaccinations are required.  Once the surgery receives the completed questionnaire the practice nurse can review your information, and the surgery will contact you to make an appointment with the Practice Nurse.

Please contact the surgery to request a questionnaire, then complete and return the travel questionnaire at least 8 weeks before you travel to allow time for the Practice Nurse to review your information, travel vaccines to be ordered and for you to receive the vaccinations at least 2 weeks before you travel. 

Please note that not all travel vaccinations are included in the services provided by the NHS, and are ordered on a private prescription which incur a charge over and above the normal prescription charge.

Some countries require proof of vaccination (for example, for polio or yellow fever vaccination), which must be documented on an International Certificate of Vaccination or Prophylaxis (ICVP) before you enter or when you leave a country.
